At registration, please bring the documentation listed below.  If these requirements are not met, teams or individual players will not be allowed to participate.

• Player Passes for Each Player

• Two copies of your State Certified Roster for Fall 2008/Spring 2009 (we keep one copy for our records)

• Medical Release Forms (Please use our Medical Release form unless yours are notarized)

• Application to Travel (only needed for outside Region I and New Jersey)

• Guest Player Approval Form
